PHQ-9 & GAD-7

All clients are required to complete the PHQ-9 and GAD-7 form at the start of their counselling, and then at six week intervals during counselling, and then at the end of their counselling.

Once you have submitted the form, the Counselling Centre will then pass on this information to your counsellor, so that they can discuss your responses with you.

To maintain confidentiality, you are asked to enter your identification number issued by the Counselling Centre instead of your name.
